Gastrik dan Ponteng Puasa
Can not fast because of gastric? Seek other ideas, not more reasons to use obsolete because studies prove that people can actually experiencing gastric problems without fasting.
Even if the concept by breaking 3-S of simple, balanced and healthy, fasting prayer would be implemented well.
Pain, indigestion or stomach discomfort during or shortly after the breaking of fast is not because of gastric, but due to eating habits and foods that fill the plate. Stomach would be capricious if you eat too much, especially if the food is kind of difficult digested.
Breaking of the recommended practice of moderate Islam and gradually eat and melambat-delay bersahur can help patients through gastric Ramadan with better and comfortable, consult your surgery Gastrointestin Tropicana Medical Center, Dr. Kwan Keat Leong.
Between gastric problems are generally very common and it can be divided into two types, namely gastritis (inflammation of the lining mucosa lining the stomach wall) and peptic ulcers in the stomach or more layers of cells in the (more severe than the gastritis).
Usually when people say they have gastric problems, it refers to the first condition (gastritis).
In nature, cells of our stomach produces hydrochloric acid (to pH 1 to pH 2) that functions digest food. Acid is a strong hand and if infected, the skin will terkakis. Tues produce stomach mucosa layer (layer of defense) as the protective effect of hydrochloric acid.
Production or secretion of hydrochloric acid occurs when there is food to begin the process of digestion and gastric occurs when there is no balance between the quantity of hydrochloric acid secretion (too much) with the strength layer of defense (weak). If this layer of defense weakened, the ulcers occur because the hydrochloric acid can reach the cells and cause inflammation.
If the person complaining abdominal pain when hungry, it indicates too much hydrochloric acid in the stomach and abdominal pain but if after eating, it may indicate problems stomach ulcers because the mucosa layer is weak.
Signs gastric easily detected because they cause discomfort in the stomach. Among the marks are:
# Stomach pain (at the top of the stomach or abdomen)
# Feeling oppressed, windy or painful
# Feeling uncomfortable
# Bloating, Sendawa repeatedly and often wind after eating
# Nausea (nausea) and vomiting
# Loss of appetite
# Weight loss (at the worst)
If you reach severe levels, causing bleeding stomach ulcer and it can be seen in two forms, namely:
# Stool becomes black as tar
# Include vomiting blood or dark coffee
Who is at risk?
Gastric problems are very common and can occur in anyone. Generally, more men suffer gastric problems with the ratio 1.5 to two times more than women. It is usually due to lifestyle and risk factors such as:
# Individuals who regularly take pain medication last in the long term, such as non-steroidal anti-inflammatory drug (NSAID). These include aspirin (taken to melt the heart patient blood)
Period of medication that allows a person experiencing gastric vary between individuals because of the sensitive, taking over several weeks can cause this problem
# Smoking and drinking a lot. Cigarette smoke and alcohol can impact negatively and harm defense layer (layer of mucosa)
# Helikobakter pilori bacterial infection (H pilori). This layer of bacteria able to weaken the mucosa in two ways, namely:
a. H pilori bacteria produce enzymes that break or disentangle urase urea to ammonia. Ammonia is toxic substances that may affect the mucosa layer. The presence of ammonia weaken many layers of defense that may cause hydrochloric acid to the cells and cause inflammation
b. H pilori form bacteria have berpilin (Twisted). So it can make holes in the mucosa layer (such as worm holes) that ultimately weaken the layers of defense is
# Pressure. Many individuals who suffer stress whether work stress, personal, social or getting infections are also more at risk of severe gastric problems attacked.
In fact, gastric problems also frequently detected in patients who undergo surgery new big because surgery is also a form of pressure to the body.
# Individuals who like to take traditional medicines, particularly those containing roots. They include traditional Chinese medicine and Malay traditional medicine because most of mixed steroids that it is more efficient and effective ‘.
Can gastric fasting patients?
“No problem. Gastric patients, especially those with gastritis, should be no problem to fast, “said Dr. Kwan.
According to him, the assumption many gastric attacks occurred more frequently in Ramadan is actually less accurate. Many believe that when fasting, the level of acidity in the stomach will increase.
During fasting, the whole digestive system rest and this means that hydrochloric acid secretion in the stomach decreased. So the problem should not arise as a result of pain or indigestion stomach acidity is high.
But that is otherwise occur because many complained abdominal pain during fasting. Dr Kwan said, the problem is actually due to eating habits during a break.
Patients should eat a little stages when the breaking of the stomach so that not a day of rest ’surprised’.
“Recommendation to the breaking dates, muih little cakes or fruits as the beginning of prayer and rest a while with very good because it gives opportunity to prepare the stomach receive more food.
“But that is otherwise occur. Many eat heavy food immediately break and this makes the problem occurrence Indigestion.
“What more food is usually the breaking of a very complex food dishes such as curry, spicy or fatty foods can cause digestive process can not run smoothly. This is why the patient felt abdominal pain after the break.
“When selecting food, always remember to practice healthy eating. Do not see the tens of arbitrary points of foods served at the buffet (if breaking in a restaurant or hotel) or when buying juadah the Ramadan bazaar. You have 30 days to try all the food.
“Maybe when hungry all see delicious food, but always remember to be moderate and not too according to desire. There is also the possibility of an upset stomach during fasting.
“Typically these patients are not due or sahur bersahur too early ie before bedtime. For those who have gastric problems, they need to eat in the morning so that no problems arise during the fasting stomach ache, “said Dr. Kwan.
According to him, stomach problems do not feel comfortable in the first week of Ramadan is normal because the stomach needs time to adapting to new circumstances (do not receive food during the day).
“This is the reason for individuals who have gastric, they have no excuse for not fasting according to the recommendations of doctors or organized religion.
“As long as they practice 3-S dietary pattern that is simple, balanced and healthy, the patient fasting gastric can run without pain,” he said.
However, for patients with stomach ulcers until bleeding occurs, up to their discretion. They can be fast but if symptoms of pain attacks, and urged the breaking of the fast break for a day or two until the symptoms disappear. But remember, even experienced bleeding ulcer patients can still be fast, taking drugs and breaking the hours of the morning.
Treatment
Dr Kwan said, individuals who have gastric problems, stomach ulcers, especially, should see a doctor four to six weeks before by Ramadan. Individuals who suffered gastric problems caused by gastritis, need to take medication twice a day for two weeks while the case for stomach ulcers, medication period is between four to six weeks.
But the problem is that often patients do not spend and medicines are usually given to stop taking their medication after three or four days when the pain disappeared.
Why should drug spending? Given the drugs actually help restore the levels of defense in the stomach and it takes two weeks to return to normal. Discontinue medication after three or four days means you deliberately gastric invited to come back.
Even better if the patient lead inspection binoculars stomach (endoscopy) to determine the level of the problem and see whether patients experienced bacterial infection H pilori to determine the appropriate medication.
There are three types of medication for gastric namely:
# Antasid – act to improve and provide an additional layer to layer of defense (mucosa) taken three times a day.
# H2 Antagonists – works to reduce acid secretion. Antasid and H2 antagonist medication given for gastritis.
# Perencat proton pump (PPI) – aimed at reducing acid secretion more effectively; usually given to patients who experience severe gastritis or stomach ulcers.
In addition to medication, patients need to maintain nutrition and lifestyle change to prevent recurrent gastric attack. Among the changes that need to do is:
# Stop smoking and drinking alcoholic beverages
# Stop the medication-resistant pain
# Practice healthy eating and regular meals (breakfast, lunch and dinner). In the month of fasting, morning and should be up-melewat miss bersahur
# Eat a little bit, but often
# Reduce food spicy, bercili, sour
# Reduce beverages containing caffeine (coffee, tea and carbonated drinks)
# Take the milk because the alkaline nature of milk can reduce stomach acidity
# Reduce intake of foods that are deemed to be ‘windy’. Type of food varies between individuals and modify according to their needs
Dietary guidelines
# Sync break and start breaking the food ‘light’, such as dates, water, fruit or low-fat soup. Avoid taking water or sweet cakes sweet.
